Chronic diseases are defined broadly as conditions that require ongoing medical attention or limit activities of daily living or both.

Some of these conditions include:

• Coronary Heart Disease
• Chronic Obstructive Pulmonary Disease (COPD)
• Hypertension

The Practice will contact those patients requiring an annual review, usually by text message or telephone, to offer an appointment with a clinician.  Please ensure your contact details are up to date.
